ORDER ON APPLICATION TO PROCEED
IN FORMA PAUPERIS
The Court, having considered Plaintiff’s application to proceed in forma pauperis
and the affidavit filed in connection therewith, finds as follows:
1. On August 26, 2026, Ms. Taylor filed an application to proceed in forma
pauperis in conjunction with a Notice of Appeal. In her application, Ms. Taylor
swore to the accuracy of the provided information under penalty of perjury.
2. Ms. Taylor indicates she has been “unemployed since 10/29/26 (sic) and
without income since Nov. 14, 2025 which was [her] last paycheck,” and that she
was “denied unemployment” after her job loss. Ms. Taylor also indicates that her
“mortgage is in acceleration.”1

3. The Court notes that, despite the fact Ms. Taylor denies the receipt of any
income or gifts from any source in the preceding twelve months,2 that she admits to
access to funds remaining from her mother’s life insurance policy to pay bills.3
Further, while denying any income of any type, Ms. Taylor outlines some monthly
expenses that are paid in some manner.
4. The Court further notes that, while Ms. Taylor lists nominal funds in
Checking and Savings accounts,4 she fails to provide any bank account information
in support.5
IT IS HEREBY ORDERED this 10th day of September 2026, that:
A. Ms. Taylor’s Application to Proceed In Forma Pauperis is DENIED
for failure to complete the affidavit fully and accurately.
B. Ms. Taylor must either pay the filing fee of $210.00 or submit a fully
